Clapham Park Group Practice Social Prescribing Service
What is Social Prescribing?
We understand that you may wish to speak to your GP about a range of things, including non-medical issues such as loneliness, unemployment or debt.
Our GPs can now refer you to our in-house Social Prescribing Link Worker, Michelle, to help with these problems.
You will be given an extended appointment so that you can discuss the problems you are facing and you will be put in touch with appropriate services such as community groups or charities who can help you. Michelle is here to listen, support your needs, and help you to start to feel better.
Michelle can:
- Listen to the challenges you are facing
- Agree an individual support plan with you
- Assist you to set goals that will empower you to make positive lifestyle changes
- Provide information and guidance on services that can help you
- Introduce you to local services, community groups, activities or options for volunteering
Michelle can help with problems such as:
- Social isolation and loneliness
- Financial worries or benefits
- Work related issues, training and volunteering
- Health and wellbeing
- Weight loss and exercise
- Bereavement
- Elderly support
- Migrant support
- Loneliness and Isolation
- Mental health (anxiety, depression, stress)
- Housing advice
